Founder and Aesthetic Specialist

Dr. Jamal Balouch

Dr. Jamal Balouch is a board-certified physician specializing in Physical Medicine and Rehabilitation (Physiatry) and Pain Management, with over 15 years of clinical experience.
He completed his training at Carolinas Rehabilitation and pursued a fellowship in Spine/Pain Management at OrthoCarolina in Charlotte, North Carolina. Before medical school, he gained research experience at UCSD Medical School and contributed to various studies and publications and graduated with a Bachelor of Science in Animal Physiology and Neurosciences.
Throughout his career, Dr. Balouch has held medical directorship roles in Acute Inpatient Rehabilitation and Spine/Pain Management across Nevada, Montana, and Northern Arizona. He also worked in Urgent Care and Workers’ Compensation at Concentra Urgent Care in North Carolina and Nevada. He is also the Medical Director of Legacy Rehab and Wellness Center.
In 2023, Dr. Balouch began developing a PM&R and Wellness clinic, which culminated in the opening of Legacy Rehab and Wellness Center in Flagstaff, AZ, in October 2024. His enduring interest in aesthetics and wellness medicine was further fueled by attending various conferences and collaborating with his sister, an experienced aesthetics nurse. Through these experiences, he witnessed firsthand the significant positive impact that aesthetic and wellness services can have on patients, solidifying his commitment to integrating these offerings into his practice. This new clinic allows him to provide comprehensive care that enhances both aesthetics medicine and overall wellness through a variety of services.

Registered Nurse

Sally Balouch

Sally Balouch is a highly experienced RN and certified advanced injector, specializing in aesthetics and skincare.
With 12 years of nursing experience and extensive training in injectables, she has become skilled in providing patients with excellent results in their cosmetic needs. Her advanced injection skills with commitment to learning new techniques have given her the ability to identify and treat each patient uniquely to their own aesthetic preferences.
With years of experience in the aesthetics field, Sally embodies a strong work ethic and takes pride in providing excellent results for her clients. Known for her calm, caring, and attentive bedside manner, she has a gift in being able to collaborate and connect with her clients and build a trusting relationship.
One of Sally’s ambitions in joining Legacy Rehab and Wellness Center is to expand the aesthetic services and acquire new skills that will help her clients on a long-term basis and impact their lives by helping them achieve their personal goals.

Angela Balouch

Angela Balouch, RN, is a registered nurse with a bachelor’s degree from the University of Wyoming and a master’s degree in jurisprudence health law from Loyola University Chicago. With over 25 years of experience in healthcare, she brings a wealth of knowledge to her role at Legacy Rehab and Wellness Center.
Angela combines her clinical expertise with a strong foundation in patient care and management, skillfully navigating the complexities of healthcare management. Her commitment to improving healthcare delivery shines through her strategic leadership and the implementation of innovative practices. Her extensive background ensures that the clinical care operations at Legacy Rehab and Wellness Center are both efficient and compassionate, fostering a patient-centered approach throughout the organization.
Angela will play a key role in managing our aesthetics and weight loss program, and in growth and expansion of our services at Legacy Rehab and Wellness Center.
Licensed Aesthetician

Marissa Fritz, LE

Marissa is a licensed aesthetician at Legacy Aesthetics, serving clients since 2022.
She graduated from the Southwest Institute of Natural Aesthetics in Tempe, Arizona, where she built a strong foundation in results-driven skincare and discovered her passion for helping others feel confident in their own skin.
She believes skincare is more than just treatments—it’s about connection, trust, and creating a space where clients feel truly seen and cared for.
As a Face Reality Certified Acne Expert, Marissa specializes in corrective acne treatments with a focus on real, lasting results. Her approach is thoughtful and personalized, guiding each client with education, intention, and support throughout their skincare journey.
In addition to acne care, she offers anti-aging treatments, chemical peels, brow service’s, and is always tailoring each experience to her clients’ unique goals.
Her goal is for every client to leave feeling confident, refreshed, and at home in their skin.
